Start with your approved process

The NDIS Commission describes incident management expectations for registered providers. Your workers need clear internal instructions explaining how to raise concerns, where to record information and whom to contact. This article does not determine whether a specific event is reportable or set notification deadlines.

Separate observation from interpretation

A useful learning exercise asks staff to identify what they directly saw or heard. Teach them to distinguish that information from an assumption about why it happened. Where a person’s words matter, record them accurately using your approved documentation approach.

Include what happened next

A record should help the appropriate reviewer understand the actions taken and any unresolved question. Avoid treating the form as the end of the process. Staff need to know how immediate concerns and further follow-up are handed over.

Use fictional examples in training

Give learners a short scenario and ask which details are missing. Do not upload identifiable participant incident reports into a training example or unapproved AI tool. Your provider should approve the example and the expected reporting pathway.

Review the learning with supervisors

Look for recurring documentation gaps and address them in a focused refresher. Care Enable can help turn your procedure into a structured lesson. Use the official Commission guidance for reportable-incident obligations and obtain appropriate advice for specific cases.

Incident documentation: a practical team exercise

Separate fact from assumption

Present a short fictional account with an observation, a person’s words and a guess about why it happened. Ask staff to separate these elements and identify missing information. Follow the approved internal process for the learning discussion.

Review these questions

  • What was directly observed?
  • What was said?
  • What action and handover need recording?

Agree one improvement, nominate the person responsible and choose when to review it. Keep the exercise separate from decisions requiring specialist advice or role-specific authorisation.
